AUTHORIZING EXECUTION OF VISION CARE SERVICE AGREEMENT BETWEEN
THE CITY AND CALIFORNIA VISION SERVICE
BE IT RESOLVED BY THE COUNCIL OF THE CITY OF ALAMEDA that the Mayor and City Clerk be
and are hereby authorized to execute and to attest tn, respectively, on behalf of the City, an
agreement with the California Vision Service, a California corporation, which provides for the
furnishing of Vision Services to certain employees of the City, said contract to be substantially
in the form of that filed in the office of the City Clerk and marked, "Filed, August 3, 1976."

I, the undersigned, hereby certify that the foregoing Resolution was duly and regularly
introduced and adopted by the Council of the City of Alameda in regular meeting assembled on the
3rd day of August, 1976, by the following vote, to wit:
