WHEREAS, a division of the State Employees' Retirement System with respect to eligjble
employees of the City of Alameda, a municipal corporation, herednafter referred. to as "Public Agency",
who ,1" mgmbers of and in positions covered by said retirement. system, has been conducted in accord-
ance with Federal and State laws and State regulations for the parposes of coveragu under the
insurance system established by Title II of the Federal '4') 9199 Security Act and a decried ratirement
system, hereinafter referred to as "(Part II), State Employees' Retirement Syste-m, has been estab-
lished dhereby as provided in Section. 218(0(8) of. the Federal Social. Security Act, composed of
positions of members of such retirement svvtem who desire coverage under. the said insurance system,
and

an agreement with the State to extend to services performed by individuals as employees of. the Public
Agency 85' members of a coverage group, as defined in Sectoion 2.18(d)(4) of the Federal Social. Security

WHEREAS, official form "Application and Agreement. Ret.-DASEDI 32D" containing the terms

NOW THEREFORE, BE IT RESOLVED BY IRE CO;ONCEL OF THE ClaiTY OF 99 9999, that said Application
and Agreement on said official. form be. Executed on behalf of the Public Agency and sabmicted to the
SLate to provide coverage under the California State Social. Security Agreement of. March. 9, 1951, of
all services performed by individuals as employees of the ?child Agency as members of a coverage
group (as defined in. Section. 218(d)(4) of 2he Social Security Act) of the said (Part II), State
Employees' Ret;rement Sygtem, except the fcgicwang:

Effective date oF coverage of services under said agreement: to be January 1, 1959; and
BE IT FURTHER REASOLATED, that H. D. Weiler, City Manager of Public Agency, City
Alameda, California, be and he is hereby atlgoriged and directed. to execute said Application and
Agreement on behalf of and as AuMbor'(. d Agent of the Public Ay,eney and to forward same to the
State for acceptance and further action, and.
BE IT FURTHER RESOLVED that authority hereafter to act as Authorized AgenL, and so to
condut all. nezotiations, conclude all arrangements, subm1 t. all reports, and sign all agreements
and Instruments which may he necessary tx, carry out the letter and intent of the aforesaid appli-
cation. and agreement, in conforzdty with all applicable Federal and State laws, rules and '4 '4
is vested in the position of an;sonnel Director of Puhlio Agency.
