Contribution Established. In the interest of the health and welfare of opployees of the City of Alameda, this Council hereby approves and authorizes a contributidn by the City, on behalf of said employeeo, in an amount of money per month net exceeding the cost thereof of group health insurance coverage for each offider and employee and one 00 el dependent of each such officer and muployee, who is eligible and who participateo under either (a) Hospital Service of California (hospital care with surgical-medical benefits), hereinafter referred to as vElue Cross Fla-rC, or (h) The Kaiser Foundation Health Plan, Inc, (Group Coverage), horeihafter referred to as Jgaiser Plan" pursuant and subject to the provisions, terms and conditions set forth below. 2, Eligibility, Except as hereinafter specified, every person, including probationers, currently employed. and receiving compensation therefor on a. full-tine permanent basis as an employee or officer of the City, and every' person thereafter entering such employment or office, is eligible to participate in thie plan. Every employee who occupios 13 . permanent ore-half time civil service position is eligible to particiTate in this plan, but the City contribution on behalf of such. employee shall be one-half the sum contributed on beheld' of other eligable participants herein. Except as nerein provibed to tne contrary, each person who is employed as a department head of the City and who is not othergise eligible hereunder, shall be eligible to partidipate in this plan in the same manner and extent as full time permanent employees and officers. '31t'. in thin 5 contrAbatton plan are (a) 04 .3 of the Bureau of' Electricity, as listed and designated separately' in an appropriate Resolution of the Department of Public Utilities of the City; (b) part-time or temporary employees, or persons holding amergency positions, of the City; (c) persons employed under contract for a definite period of time and for the performance of specific duties or servioes; and (d) all positions under the Board of Education. 5. Participation'. Every euigible employee, on or before the effective date hereof, shall elect whether 02 not to participate in this plan. Said election shall be in the form prescribed by the City Manager, and shall include payroll deduction authorization for the balance of the cost in excess of the amount herein. authorized as the City's contribution per month for coverage under either Blue Oroas or Raiser Plan- Eseh such electing employee :shall become a participating employee here- under upon the earliest date thereafter permitted under the terms, conditions and provisions of the respective Blue Cross Plan or Kaiser Plan, and the processing requirements of the Auditor. Any person who may- hereafter became an eligible euftEmie, as defined hereinabove, may elect to participate and shall become a participating employee under this plan of contritutien in the same manner. 4. CoragYe. (a) dbis plan for the health ahl welfare of eligible employees and officers shall become effective for coverage beginning November 1, 1969. Payroll deductions therefor shall commence in the preceding pay period. Those eligible emplinyees Eno are or (1 later become members of Blue Cross Plan or Kaiser Plan may transfer from one plan or coverage to the other, or daphasayae partiaipablmu membership or coverage, only at suah timos as may be permitted under the terms, conditions and provisions of such respective Plons, (b) Bequests for adjuatments complaints, questions of coverage and all similar adminis- rative matters shall be raised by the affected participating employee directly with the Moe Cross Plan or the Kaiser Plan as the case may be. 5. Conditions, (a) The contribution herein directed shall. ba no event be paid to the employee. Such contribution per participating employee shall. De paid by the Auditor only to Blue Cross Plan or Kaiser Plan, as appropriate, together with the balance of the cost of coverage for each such employee deducted. from Etch empleyeejs (ay' as authorized by ouch employee. (b) Said contribution per participating employee shall not be deemed az, nor included in, gross income of such employee, nor shall it be construed to increase or alter pension benefits paid under any City retirement Or pension system. (c) Every employee or officer who does not participate in this :Oath or who does not continuo ouch participation. once having joined or acquired such health iheurance coverage, by member- shiT ih the Blue Cross Plan or Kaiser Plan and by payroll deductions for payment of the cost of coverage, shall not be eliglhle for, and shall nut receive, payment of such contributien for or on his or her behalf, and shall have no claim to payment or receipt of such sum from the City. (d) The Auditor ehall make one such contributory payment of the amount hereinabove specified for and on behalf of every participating employee who is on leave of aboence without pay for the first calendar month follovdng the comaencement of ouch leave, and thereupon such payments shall. terminate. (e) Cessation of she City employment of any participating employee snail terminate on the date thereof sufa payments by the Auditor for and on behalf of such employee. (f) Any participating employee who terminates City employment, or Who retires, or who is on leave of absence without pay, or who is on sick leave without pay, shall arrange directly with Blue Cross Plan or Kaiser Plan, as the case may be, for continuation of coverage and payment therefor, if' such coutinuation is desired by- said amployee and if such umpleyee is eligible under the teruu, conditions and provisions of sunh plan for continued coveruge. O. Repeal of Prier Contributions Authorizatien. Resolution, No. 6854, adopted October 5, 1965, is hereby repealed to the extent it conflicts with or duplicates any of the terms herein contained- BE IT FURITIIi RESOLVED that the City- Manager ib authorized and directed to execute, and the City Clerk to attest, on behala' of khe City contracts with said Hospital Service of California, and The Kaiser Foundation Health.
