AUTHORIZING EXECUTION CF AMENDMENT TO DENTAL CARE SERVICE
AGREEMENT BETWEEN CITY AND CALIFORNIA DENTAL SERVICE
BE IT RESOLVED BY THE COUNCIL OF THE CITY OF ALAMEDA that the Mayor and City Clerk be
and each hereby is authorized to execute and to attest to, respectively, on behalf of the City
an amendoment, to be effective July 1, 1975, to that certain dental care service agreement with
the California Dental Service, a California corporation, heretofore executed by said parties
pursuant to Resolution No. 8150, adopted January 2, 1974, said amendment to be substantially in
the form of that contract denominated, "Amendment to Agreement," filed in the office of the City
Clerk and marked, "Filed, May 27, 1975."

I, the undersigned, hereby certify that the foregoing Resolution was duly and regularly
introduced and adopted by the Council of the City of Alameda in regular meeting assembled on the
3rd day of June, 1975, by the following vote, to wit:
AYES: Councilmen Beckam, Diament, Hurwitz, Sherratt and President Corica, (5).
