AUTHORIZING EXECUTION OF DENTAL CARE SERVICE CONTRACT
BETWEEN CITY AND CALIFORNIA DENTAL SERVICE AND
DESIGNATING PERSONNEL DIRECTOR AS GROUP REPRESENTATIVE
BE IT RESOLVED BY THE COUNCIL OF THE CITY OF ALAMEDA that
the Mayor and City Clerk be and each hereby is authorized to
execute and to attest to respectively, on behalf of the City,
a dental care service contract with California Dental Service,
a California not- for - profit corporation, which provides for the
furnishing of dental services to certain employees of the City,
said contract to be effective July 1, 1978 and to be substan-
tially in the form of that filed in the office of the City Clerk
and marked, "Filed, May 16, 1978."
BE IT FURTHER RESOLVED that D. Glenn Tarver, Personnel
Director of the City of Alameda, is hereby designated the Group
Representative of the City, to be responsible for providing
evidence of coverage to eligible employees and disseminating
all notices delivered by California Dental Service to the City.
