AUTHORIZING EXECUTION OF AGREEMEW WITH OCCUPATIONAL
HEALTH SERVICES, INC. FOR CONTINUATION or THE EMPLOYEE
ASSISTANCE PROGRAM
WHEREAS, a proposed agreement with Occupational Health
Services, Inc., for a continuation of its services in connection
with the employee assistance program, has been submitted to this
Council, and the Council is fully informed and aware of the terms
and conditions thereof;
NOW, 7HEPEFORE, BE IT RESOLVED BY THE COUNCIL OF THE CITY
OF ALAMEDA that the City Manager and City Clerk be, and each
hereby is, authorized and directed to execute and to attest to,
respectively, on behalf of the City, an agreement for Employee
Assistance Program Services with Occupational Eealth Services,
Inc., said agreement to be substantially in the form of that on
file in the office of the City Clerk.
L, the undersigned, hereby certify that the foregoing Resolution
was duly and regularly adopted and passed by the Council of the
City of Alameda in a regular meeting assembled on the 3rd day of July,
1984, by the following vote to wit:
AYES: Councilmembers Gorman, Hanna, Lucas, Monsef and President Diament - 5.
ROES: None.
ABSENT: None.
