AUTHORIZING EXECUTION OF AMENDMENT TO SERVICE AGREEMENT
WITH GATES, McDONALD & COMPANY FOR CLAIM ADMINISTRATION
OF THE SELF-INSURED WORKERS' COMPENSATION PROGRAM OF
THE CITY AND BUREAU OF ELECTRICITY
WHEREAS, on November 1, 1976 the City entered into an
agreement with Gates, McDonald & Company for its services in
the administration of the self-insured workers' compensat on
program of the City and the Bureau of Electricity; and
WHEREAS, an amendment to said agreement has been submitted
to amend the term and the consideration provisions thereof;
NOW, THEREFORE, BE IT RESOLVED by the Council of the City
of Alameda that the City Manager and City Clerk be, and each
hereby is, authorized to execute and to attest to, respectively,
on behalf of the City, an amendment to the Service Agreement
with Gates, McDonald & Company, providing for the extension of
said agreement to October 31, 1985 and providing for self-renewal
on a year-to-year basis thereafter unless terminated as provided
in said agreement, and amending the service fee for the contract
„)z7,,rlf year, payable quarterly at the rate of $4,300.00.
I, the undersigned, heresy certify that the foregoing Resolution
was duly and regularly adopted and passed by the Council of the
City of Alameda in regular meeting assembled on the 16th_ of October, 1984,
by the following vote to wit:
AYES: Councilmembers Gorman, Hanna, Lucas, Monsef and President Diament - 5.
NOES: None.
ABSENT: None.
