THE AUDITOR. TO MAKE PAYROLI DEDUCTIONS FOR
PREMIUMS.
WHEREAS, a large number of employees of the City. of Alameda are desirous
of securing for themselves and. their dependents the benefits of group hospital,
surgical and medlcal care insurance under a plan of group insurance offered by the
Hospital Service 011 California. Blue Cross Plan); and
niCGI-.)AS, in the opinion of this Council the proposed. group insurance
plan would be beneficial to said employees and to the public service in F,eneral;
NOW, THEREFORE, BE IT RESOLVED BY THE COUNCIL 01? THE CITY OF ALAMEDA
that the City Manager of the City of Alameda is hereby authorizedsand(irected
to make application to the Hospital Service of California (Blue Cross Flan) for
group hospital, surgical and medical inburanoe covering tic' employees of the City
of Alameda and. their dependents, and to execute, for and on behalf of t•e City' of
Alameda, any' and all applieationb, forms and •ot 100 papers or documents necessary
to obtain and caube such. group insurance to become effective.
RESOLVED, JEUZIGIFR, that the Auditor of the City of Alameda. is hereby
authorized and directed to nuke payroll deductions front the salaries 0 wages of
those employees who authorize, In writing, such deducti( ns, in.. sums equal to the
ainpunt of pramiums to be paid. by each. such employee i0 .. accordance with. the sche-
dule of premiums specified in thp group policy issued by. said company.
